| The Case Manager Hospice is responsible for primary case management, including direct patient care, of patients inthe home setting. Documents care given and communicates with other health-care team members. BSN preferred. All (1) newly graduated nurses, (2) re-entry nurses, and (3) nurses new to the U.S. healthcare system must satisfactorily complete the CVHP R.N. Residency Program within the first 6 months of employment at CVHP. Minimum of one-year experience as a nurse in either acute care setting, home care or Hospice. Excellent customer service skills required. Current, valid Drivers License and current liability insurance required. Certification preferred specialty area. Current California RN license. BLS required; Good IV skills, strong interpersonal skills. |
