Working in collaboration with the Chief Patient Support Operations Officer, the Manager of Palliative Care is responsible for the oversight of the provision of palliative care to patients in the hospital and patients receiving palliative care post discharge. Works in a consultative manner with multidisciplinary care team to address pain and symptom management, advanced planning, emotional support, and guidance on the goals of care. Provides education and information on palliative care issues to patients/families/caregivers. Actively participates in multicultural palliative education of health care professionals. Provides additional, culturally competent psychosocial support to the patient/family/caregiver. Provides proactive intervention within the physician's plan of care to help the patient/family/caregiver with end of life care options and resources. This is a working manager position with manager seeing patients and cross covering for nurses. Contributes to the smooth team functioning of the department as a member of the patient care team. BSN required, Masters or NP preferred. Bilingual in Spanish is preferred. Minimum of two years experience in a leadership position. Minimum of two years as a nurse in palliative care. Computer skills are preferred. Excellent customer service skills required. Must have valid current drivers license. CHPN credential (Certified Hospice and Palliative Nurse)required within three months of hire. Current California RN license.
