The Case Manager Home Health is responsible for primary case management, including direct patient care, of assigned patients in the home setting. Documents care given and communicates with other health-care team members. Coordinates and implements patient care specific to the age of the patient. Ensures quality of care is provided in efficient and safe manner. Accepts responsibility of patient care from patient's admission to discharge.

BSN preferred.

All (1) newly graduated nurses, (2) re-entry nurses, and (3) nurses new to the U.S. healthcare system must satisfactorily complete the CVHP R.N. Residency Program within the first 6 months of employment at CVHP. Minimum of one-year experience as a nurse in a home health setting. Frequent driving. Minimum of one year acute care experience is required. Excellent customer service skills required.

Current, valid Drivers License and current liability insurance required. Certification preferred in specialty area. Current California RN license. BLS required. Good IV skills, computer skills, strong interpersonal skills.
